Haemorrhoids are commonly known as piles. Piles are the commonest ano rectal disorder. Commonest clinical manifestations of piles are bleeding while passing motion. More than 50% of the population develops piles but only 10 to 20% needs surgical treatment. The high incidence of piles and the non specificity of the clinical symptoms need a precise differential diagnosis, in order to exclude other disease of the colon like cancer rectum. We can see the causes, signs, symptoms, investigations and recent treatment for piles.

What is piles ? How many types ?

Piles are varicosities of haemorrhoidal venous plexuses in the anal region. They are of two types known as internal and external piles.

What are the reasons for piles?

These are the common reasons for piles

[a]. Sitting too long time in the toilet.

[b]. Straining while passing motions (constipation)

[c]. Low fibre diet

[d]. Familiarity

[e]. Obesity

[f].  Chronic diarrhea, Dysentry.

[g]. Pregnancy

[h]. Cancer rectum

[i ]. Cirrhosis with portal hypertension.

[j].  Erect posture

One author said piles are “ the penalty paying by the human race for the upright posture”. Hence Piles are not developed in animals.

What are the clinical signs and symptoms of piles ?

Symptoms may vary with condition such as pregnancy, constipation, repeated straining, changes in diet or work pattern. Commonest symptom is  bleeding. Associated with reducible lump or mass, discharge, itching and burning sensation.

Suppose if a person is getting one or all of the symptoms mentioned above, he should consult the doctor for the proper diagnosis and treatment. Sometimes other disorders like cancer rectum can produce all those symptoms.


How to diagnose piles ?

Even though piles are the most common source of rectal bleeding it is imperative that other causes to be excluded. Since piles are easily diagnosed by proctoscopy, it is mandatory to do colonoscopy or sigmoidoscopy in high risk patients above 50 years to exclude other source of rectal bleeding. Eg. Cancer rectum

What are the various degrees of piles?

Piles are divided into four degrees.

[a]. 1st Degree           : First degree piles cause painless.

[b]. 2nd Degree           : Pile mass comes out while passing motion but

goes in on its own.

[c]. 3rd Degree           : The prolapse must be reduced manually.

[d]. 4th Degree           :  Prolapsed piles stays out all the time cannot

be manually reduced.

What are the common tests to be done?

1. Sigmoidoscopy

2. Colonoscopy

3. USG abdomen

4. Blood tests, X-ray chest, ECG.

What are the treatment available for piles?

Treatment for piles depends upon the degree of piles.   

A]. For first degree piles
Diet modification is very important.  Constipation can be avoided by taking more fibre diet like green vegetables, fruits, wheat products, intake of more amount of water, laxatives can be used for free motion,  Laser [or] infrared coagulation also used for who fails conservative treatment.

B]. For second degree piles.

Apart from diet modification and laxatives, Laser, rubber band ligation, infrared coagulation and sclerotherapy are available treatment.

C]. What is the treatment available for 3rd and 4th degree piles?

Surgery is the only option for this late stage piles. It may be conventional surgery, LASER procedure or Stapler method.

D]. What are the advantage of laser over conventional surgery?

Laser is used for all stages of piles.  LASER PHOTOCOAGULATION, is done for First and Second degree piles and it shrinks the pile mass.

For third and fourth degree LASER PROCEDURE is better treatment than conventional surgery.

The advantages of laser procedure over conventional surgery are [a] less pain [b] Less bleed [c] shorter hospital stay [one day] [d] Early return to normal activity [e] Low cost

So for i have done more than 6,000 Laser procedures successfully.

What is the recent treatment for piles.

Recent treatment for haemorrhoids is STAPLER METHOD.

In this method the specially deviced haemorrhoidal stapler (PPHO3) is used. This technique reduces the prolapse by a circular excision of rectal mucosa with a stapling device.

The advantages: 
[a] Minimal pain [b] Very minimal blood loss [c] No post surgical discomfort [d] Short stay in the hospital [ one day].
[e] Early return to the normal activity [f] No anal stenosis.

For these all advantages, we can say, STAPLER METHOD is a modern technique for piles problem.

So for i have done more than 400 cases very successfully.

How to prevent development of piles?

Though various treatments are available for piles,
Prevention is better than cure. Constipation is commonest cause for piles. It can be avoided by
  • Changes in diet and life style.
  • Eat more vegetables and fruits [ Fibre diet]
  • Drink more water.(2.5 to 3 liters per day)
  • Laxatives, stool softeners.
  • Avoid straining while passing motion.
  • Regular exercise.

Both lasers and stapler haemorrhoidopexy are the best methods for piles treatment and offers combination of low cost, quality care, minimally invasive and patient compliance procedure.
